Job Description
Basic Purpose of the Job
Practices at an advanced clinical level and provides comprehensive pharmaceutical care through independent clinical decision making, medication therapy management, quality improvement, interdisciplinary collaboration, and professional mentoring. Serves as an experienced clinical pharmacist who supports departmental initiatives, advances medication safety, and contributes to the education and development of pharmacy personnel and other healthcare professionals.

Job Requirements
- Minimum Education: Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy with health system training.
- License / Certification Required: Active Tennessee pharmacist license required.
Minimum Work Experience: Minimum three (3) years of relevant pharmacy practice experience; or Successful completion of an ASHP accredited PGY1 residency.
Preferred Qualifications
- Board certification appropriate to practice area.
- Experience leading quality improvement or medication safety initiatives.
- Experience precepting pharmacy students or residents.
- Demonstrated leadership in interdisciplinary practice.

- Advanced knowledge of pharmacotherapy and medication use systems.
- Advanced clinical decision making and therapeutic problem solving skills.
- Proficiency in pharmacokinetic dosing, therapeutic drug monitoring, and medication management.
- Ability to lead projects, analyze clinical data, and implement practice improvements.
- Effective communication, presentation, and interdisciplinary collaboration skills.
- Ability to educate and mentor pharmacy personnel and healthcare professionals.
Demonstrates leadership, professionalism, accountability, sound clinical judgment, and the ability to influence pharmacy practice.
Duties and Responsibilities
Performs all duties and responsibilities of Pharmacist I.
Provides comprehensive medication therapy management, pharmacokinetic dosing, therapeutic drug monitoring, and clinical consultative services in accordance with departmental policies and collaborative practice agreements.
Collaborates with physicians, advanced practice providers, nursing, and other healthcare professionals to optimize medication therapy and improve patient outcomes.
Leads or participates in medication safety, quality improvement, formulary management, operational excellence, and departmental performance improvement initiatives.
Collects, analyzes, and reports clinical and operational data to support quality improvement, medication safety, and Pharmacy and Therapeutics Committee initiatives.
Develops and delivers educational programs, competency activities, and in-service training for pharmacists, pharmacy technicians, students, residents, nursing staff, and other healthcare professionals.
Serves as a preceptor for pharmacy students, pharmacy residents, pharmacists, and other learners while supporting professional development within the department.
Assists with development and implementation of departmental policies, clinical guidelines, order sets, and evidence based medication use practices.
Maintains competency in both distributive and clinical pharmacy practice while serving as a clinical resource for pharmacists, technicians, providers, and nursing staff.
Performs other duties and responsibilities as assigned in support of departmental and organizational objectives.

Heavy lifting, pushing, and pulling is required up to 50 lbs occasionally, 20-30 lbs frequently, and 10lbs constantly to move objects of up to 150 lbs. Sudden emergency situations have the potential for exposure to lifting or moving of up to 150 lbs. Frequent bending, walking, sitting squatting, reaching, and standing are required. Keyboard/computer use and/or repetitive motions may be required.
