Key Responsibilities

  • Organizes and participates in meetings related to care coordination; completes documentation for direct services provided to ensure timely and accurate completion.
  • Transports individuals receiving services to appointments, activities, and/or work.

Requirements

  • Minimum Job Requirements: High School diploma or equivalent and one year of experience working in a human services field are required.
  • A valid driver's license with acceptable driving record is preferred.
  • Experience working with adolescents in a behavioral health setting is preferred.
  • Minimum Shifts: Candidates are required to cover a minimum of 1 shift per month.

Benefits & Compensation

  • Hourly rate: $1.5/hr.

Work Schedule

  • Per Diem position.
  • Call responsibilities required.
  • Night or evening shifts may be required.
