The Nurse Practitioner or Physician Assistant will provide personalized primary care to seniors and individuals with complex medical needs in their home environments. They are responsible for conducting clinical assessments, developing care plans, and collaborating with interdisciplinary teams to improve patient outcomes.
Candidates must hold an active state license as a Nurse Practitioner or Physician Assistant, with a Master's degree in Nursing required for NPs. A minimum of five years of nursing experience is required, with at least two years of experience in an NP or PA role preferred.
