Key Responsibilities

Provide case management and follow

up care to support patients in meeting health and treatment goals.

Collaborate with primary care and behavioral health providers to coordinate care, treatment plans, and patient education related to mental health, SUD, and psychosocial needs.

Serve as a liaison across the integrated care team to improve communication and coordination of services.

Requirements

* Licensure Requirements: Current Wisconsin licensure as a Certified Social Worker (CSW) or Advanced Practice Social Worker (APSW), or eligibility to obtain licensure within six (6) months of hire.

License must be active and in good standing with the State of Wisconsin.
