The Clinical Pharmacist provides comprehensive pharmacy services for home infusion therapy patients, including prescription review and individualized care planning. They also act as a clinical resource for the healthcare team and ensure accurate medication preparation and documentation.
Candidates must hold a Bachelor of Science degree from an accredited pharmacy school and possess a valid Registered Pharmacist license. Two years of experience in intravenous admixing is preferred, along with knowledge of HIPAA and JCAHO standards.
