Key Responsibilities

• quality, comprehensive primary care to infants, children, and adolescents in both an FQHC clinic and affiliated school

• based health centers. This role ensures the delivery of evidence

• based, patient

• Provide primary pediatric care including preventive services, chronic disease management, developmental screening, and behavioral health integration.

Requirements

• Ensure compliance with Joint Commission standards and participate in preparation and audits.

• Contribute to quality improvement initiatives and performance metrics, including clinical quality measures (CQMs).

• Participate in peer reviews and case conferences. Team Collaboration and Training:

• Collaborate with a multidisciplinary care team including nurses, behavioral health providers, social workers, and community health workers.
