The Hospice Clinical Social Worker assists the interdisciplinary team in assessing the social, emotional, and spiritual needs of patients and their families. They develop and implement care plans, provide grief counseling, and facilitate discharge planning to ensure patient dignity and well-being.
Candidates must hold a Master's degree in Social Work or a related field and possess a valid license to practice in the state of operation. At least one year of professional post-graduate social work experience in a healthcare setting is required.
