OB Summary

A Registered Physical Therapist (PT) administers physical therapy to patients in their place of residence and is performed in accordance with physician orders and IDG plan of care under the direction and supervision of the Director/Manager of Patient Services.

Key Responsibilities

  • Develop and update individualized care plans
  • Provide physical therapy services per physician’s plan of care
  • Assess functional status, mobility, and home environment
  • Educate patients and families on therapy techniques and equipment
  • Maintain accurate documentation and communicate progress to physicians

Requirements

  • Licensed Physical Therapist in the State of Colorado
  • Degree from a CAPTE-accredited program; passed state-approved exam
  • Minimum 2 years’ experience (home health preferred)
  • Current CPR certification
  • Valid driver’s license, insurance, and reliable transportation
  • Grand Junction salary: per visit model
  • $ 87,000.00 - $115,000.00
