Job Details: Job Location: Home Care - Muskegon, MI, Salary Range: Undisclosed, RN Case Manager - Full Time, 40 hours per week - day shifts with rotating weekends and holidays QUALIFICATIONS Registered Nurse with degree from an accredited nursing school and holds a current license in the State of Michigan.
Possesses and maintains a current CPR certification. Maintains a valid Michigan driver’s license and has the availability of a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.
SUMMARY OF JOB RESPONSIBILITIES The Case Manager provides skilled professional nursing care to the patient/family/caregiver as prescribed by the physician and in compliance with regulations as established by the State Board of Registered Nursing, the patient and the policies of Harbor Hospice. Case Manager creates & oversees Plans of Care for each patient admitted to Home Care.
