Job Description
The RN Clinical Navigator will be responsible for providing clinical care and guidance to high-risk patients in order to promote better health outcomes.
The position will work to eliminate barriers to care by guiding, educating and navigating patients to receive necessary health care monitoring and resources, both within Centra and externally.
The position will be charged with building professional relationships with the patient and their families, local and regional healthcare providers and vendors to ensure smooth transitions and timely access to care for patients.
The practice of the professional nurse reflects the American Nurses Association's Code of Ethics for Nurses and the Nurse Practice Act of Virginia.

The RN Clinical Navigator provides clinical care and guidance to high-risk patients to improve health outcomes and eliminate barriers to care. They build professional relationships with patients, families, and healthcare providers to ensure smooth transitions and timely access to resources.
Requirements
Candidates must be graduates of an accredited nursing program with at least three years of nursing experience and an active Virginia RN license. A current American Heart Association Basic Life Support (AHA BLS) certification is also required.
