Social Worker (LSW) - Outpatient Dialysis Center

Key Responsibilities

The Social Worker will conduct psychosocial assessments and develop goal-directed care plans for patients in an outpatient dialysis setting. They will also provide crisis intervention, patient advocacy, and facilitate referrals to community resources to support patient adjustment and well-being.

Requirements
