Nurse Practitioner Palliative Care

Key Responsibilities

The Nurse Practitioner provides clinical direction and palliative care to patients, collaborating with the interdisciplinary team to develop and implement care plans. Responsibilities include conducting home visits, performing face-to-face assessments, and educating patients and families on disease processes and end-of-life care.

Requirements

Candidates must hold a degree from a Baccalaureate School of Nursing and a Nurse Practitioner focused Master’s or Doctoral program, along with current state licensure as a Registered Nurse and Advanced Practice Nurse. A minimum of two years of experience in an advanced practice clinical setting, preferably in hospice or home health, is required.
