Home Health Physical Therapist (PT) - Contingent

Key Responsibilities

Provides skilled physical therapy treatments to patients in their homes according to a physician's plan of care. Collaborates with the healthcare team to assess functional levels, set goals, and instruct patients and caregivers on injury management and equipment use.

Requirements

Requires an active Michigan Physical Therapy license, a valid driver's license, and reliable transportation. Previous home health care experience is preferred.
