Key Responsibilities

Conduct psychosocial assessments and assist patients in coping with stress related to hospitalization and chronic illness. Collaborate with the Clinical Care Management team to determine appropriate post-hospital care levels and coordinate community resource referrals.

Requirements

A Master's in Social Work is required, with an LCSW and state certification preferred. At least one year of professional experience in a hospital or health-related setting is preferred.
