Key Responsibilities
- Employs counseling skills in order to help patient/families understand, accept and follow recommendations for care and to restore patient to optimum social and health adjustment.
- Stays up-to-date on current trends and researches the availability of community agencies and community resources for social, emotional, and financial assistance.
- Intervenes in crisis situations, and effectively applies crisis intervention theory techniques.
- Identifies and records clinical, psychosocial and financial barriers to a smooth transition across the healthcare network.
