The Nurse Practitioner will conduct face-to-face hospice eligibility visits and manage terminal illness through individualized care plans. They will collaborate with the interdisciplinary team to oversee patient care, prescribe treatments, and provide counseling on end-of-life issues.
Candidates must hold a master's degree and be a graduate of an accredited Nurse Practitioner program with current Hawaii licensure. A minimum of one year of experience in both Nurse Practitioner practice and hospice or palliative care is required.
