Key Responsibilities

The Physical Therapist will conduct comprehensive evaluations and develop individualized treatment plans to improve patient mobility, strength, and functional independence in their homes. They are responsible for providing skilled interventions, educating caregivers, and maintaining accurate clinical documentation in compliance with Medicare standards.

Requirements

Candidates must hold a degree from an accredited Physical Therapy program and possess a current Michigan Physical Therapist license. Strong clinical assessment skills and reliable transportation are required, with home health experience preferred.
