Hospice Medical Social Worker

Key Responsibilities

Deliver compassionate, patient-centered hospice care by conducting psychosocial assessments and developing individualized care plans. Provide emotional support and counseling to patients and families while collaborating with an interdisciplinary team.

Requirements

Requires a Master's degree in Social Work from an accredited graduate school and one to two years of post-graduate healthcare experience. Candidates must possess a valid driver's license and the ability to travel to patient homes.
