About the Role
Join healthscie as a Pharmacist in United States.
Key Responsibilities
- Perform assessments of all medication therapies prescribed for HSN patients.
- Provide oral and written follow-up with treatment recommendations to the prescriber as required.
- Provide medication reconciliation services and interact with patients/families and other health care professionals with regards to patients’ home medication usage.
- Review medication orders for all HSN patients and ensure safe and timely provision of medication distribution.
Requirements
- EDUCATION AND TRAINING: 1.
- Minimum of a four (4) year Bachelor’s Degree in Pharmacy, from an accredited university.
- Doctorate of Pharmacy (PharmD) is preferred with a residency program recognized by the OCP.
- Current Certificate of Registration in good standing with the OCP.
