Key Responsibilities
- Provide ongoing monitoring, chronic disease management, and follow
- up care for clients. Respond to episodic illnesses, complex cases, and crisis situations, ensuring timely interventions.
- Maintain accurate and comprehensive documentation in electronic medical records (EMRs), participate in chart reviews, and engage in case conferencing. Interdisciplinary Collaboration & Coordination
- Work closely with interdisciplinary team members and external service providers to ensure coordinated, holistic care.
Requirements
- BScN with Nurse Practitioner certification and registration with the Ontario College of Nurses (Extended Class). Experience & Skills
- based nursing experience with strong clinical, assessment, and problem
- solving skills.
- Demonstrated ability to work effectively in primary health care, particularly in community health settings.
