Key Responsibilities

Provide ongoing monitoring, chronic disease management, and follow

up care for clients. Respond to episodic illnesses, complex cases, and crisis situations, ensuring timely interventions.

Maintain accurate and comprehensive documentation in electronic medical records (EMRs), participate in chart reviews, and engage in case conferencing. Interdisciplinary Collaboration & Coordination

Work closely with interdisciplinary team members and external service providers to ensure coordinated, holistic care.

Requirements

BScN with Nurse Practitioner certification and registration with the Ontario College of Nurses (Extended Class). Experience & Skills

based nursing experience with strong clinical, assessment, and problem

solving skills.

Demonstrated ability to work effectively in primary health care, particularly in community health settings.
