Hospice RN Case Manager
Job Description
About the Role
Optimal Care Hospice Registered Nurses provide comprehensive nursing care, case management, and compassionate support to terminally ill patients and their families in home or facility settings. You will assess, plan, coordinate, implement, and evaluate individualized patient care plans.
Key Responsibilities
- Conduct initial patient evaluations and ongoing reassessments.
- Manage caseload including assessment, planning, coordination, implementation, and evaluation.
- Perform hospice-specific assessments including home safety and support system evaluations.
- Provide compassionate support to patients and families, including at the time of death.
- Conduct bereavement assessments as requested.
- Maintain positive and professional communication with physicians, patients, families, facilities, visitors, and other team members.
Requirements
- Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
- Current State License as a Registered Nurse.
- Minimum one (1) year experience as a nurse in a clinical care setting.
- Nursing skills as defined by accepted nursing standards.
- Demonstrates ability to assess and respond to the needs of patients, families, and caregivers in varied settings.
- Reliable transportation and valid automobile insurance coverage.

- Salary range: $80,000 - $85,000 annually.

Work Schedule
- Regular hours: 8:00 AM–5:00 PM, Monday through Friday.
