Key Responsibilities
- Promptly meet with clients and caregivers on a regular basis to further treatment goals utilizing culturally informed and best practice modalities.
- Coordinate with multi
- disciplinary teams for the purpose of care planning and support.
- Collaborate with agency staff, caregivers, other professionals and interested parties to formulate and implement individual client care plans, behavioral programs, and interventions.
