Hospice Registered Nurse (RN) Case Manager

Key Responsibilities

The RN Case Manager provides clinical oversight and advocacy for hospice patients by conducting assessments and managing individualized plans of care. They coordinate services, educate families, and maintain accurate clinical documentation to ensure high-quality end-of-life care.

Requirements

Candidates must be graduates of an accredited nursing school and hold a valid Registered Nurse license in Rhode Island. A minimum of 1-3 years of clinical experience and strong assessment and communication skills are required.
