Key Responsibilities

Provide one-on-one physical therapy to older adults in private residences and senior living communities to improve mobility and independence. Responsibilities include performing evaluations, developing individualized care plans, and utilizing telehealth services when appropriate.

Requirements

Candidates must hold a valid Physical Therapy license in the state of practice and a degree from an accredited Physical Therapy program. Current CPR certification and basic computer/EMR proficiency are also required.
