About the Role
Remote Care Partners seeks a full-time Nurse Practitioner licensed in Florida and Mississippi to provide virtual chronic care management and remote patient monitoring.
Key Responsibilities
- Conduct in-person initiating visits and virtual follow-ups.

Requirements
- Active NP licenses in Florida and Mississippi.
