Key Responsibilities

Coordinate the interdisciplinary plan of care for patients from admission through discharge while integrating patient and family preferences. Monitor patient care through assessments and medical records to ensure appropriate documentation and service delivery.

Requirements

Requires a clinical counselor or social worker credential from the Illinois Department of Financial and Professional Regulation. Candidates must possess at least a high school diploma with two years of experience or an associate/technical degree.
