Key Responsibilities

• Deliver reablement‑focused OT assessments and interventions.

• Maintain accurate case records on Mosaic (daily updates required).

• With this information, we will provide appropriate support to you throughout the process and into your work placement.

Requirements

• To be successful in this role you must have,.

• HCPC‑registered Occupational Therapist.

• Strong knowledge of Adult Social Care legislation and OT practice.

• Experience achieving positive outcomes through MDT working.
