Join Transition Home Healthcare as a Physical Therapist Assistant (PTA) - Full Time - Northeast in United States. Transition Home Healthcare is an elite home health agency serving Houston, TX and surrounding areas since 2010. We are a group of passionate people dedicated to delivering the highest quality services to our patients, families, team members, and the community.

Key Responsibilities

Assist in the facilitation of rehabilitation of patients with emotional, physical or mental disabilities.

Assist in the implementation of physical therapy care plans.

Assist with the coordination of care with the home healthcare team.

Educate patients and family members about health conditions.

Requirements

Assistant Physical Therapist in the state of Texas.

Home health experience highly preferred.

Knowledge of home health regulations and general healthcare compliance.

Experience with HCHB EMR. (preferred but not required)
