Join Transition Home Healthcare as a Physical Therapist Assistant (PTA) - Full Time - Northeast in United States. Transition Home Healthcare is an elite home health agency serving Houston, TX and surrounding areas since 2010. We are a group of passionate people dedicated to delivering the highest quality services to our patients, families, team members, and the community.

Key Responsibilities

  • Assist in the facilitation of rehabilitation of patients with emotional, physical or mental disabilities.
  • Assist in the implementation of physical therapy care plans.
  • Assist with the coordination of care with the home healthcare team.
  • Educate patients and family members about health conditions.

Requirements

  • Assistant Physical Therapist in the state of Texas.
  • Home health experience highly preferred.
  • Knowledge of home health regulations and general healthcare compliance.
  • Experience with HCHB EMR. (preferred but not required)
