Manage a defined patient panel by performing clinical assessments, formulating care plans, and coordinating with interdisciplinary teams to improve health outcomes. Provide longitudinal care across multiple settings including patient homes, telehealth, and clinics to reduce hospitalizations and manage chronic conditions.
Requires a Master’s degree in Nursing and at least 2 years of experience as a Nurse Practitioner with a focus on cardiology or nephrology. Candidates must hold current state licensure, national board certification, and a valid driver's license for regional travel.
