Manage a defined patient panel by performing clinical assessments, formulating care plans, and coordinating with interdisciplinary teams to improve health outcomes. Provide longitudinal care across multiple settings including patient homes, telehealth, and clinics to reduce hospitalizations and manage chronic conditions.
Requires a Master’s degree in Nursing and at least 2 years of experience as a Nurse Practitioner in primary care, cardiology, or nephrology. Candidates must hold a current state license, national board certification, and a valid driver's license for regional travel.
