The Nurse Practitioner manages a defined patient panel, performing physical exams, ordering diagnostic tests, and formulating longitudinal care plans for patients with chronic conditions. They collaborate with interdisciplinary teams to ensure care alignment and provide clinical support across home, telehealth, and clinic settings.
Candidates must hold a Master’s degree in Nursing and possess at least 2 years of experience as a Nurse Practitioner in cardiology or nephrology. A current state license, national board certification, and a valid driver's license are required.
