Key Responsibilities

• Develop a plan of care and group protocols to help individuals and groups address barriers to occupational participation via person and occupation-centered interventions.

• Lay out the types of activities and specific goals to be accomplished which will be incorporated in the individual’s person-centered plan 4.

• Document goals, applicable interventions and progress adhering to reimbursement and practice act standards.

Requirements

• Maintain all necessary documentation and records in a timely manner.

• This may include the use of an Electronic Health Record system (EHR).

• Demonstrate knowledge of and comply with all agency policies and procedures, as well and service definitions related to specific programs.

• Complete all other relevant responsibilities as assigned by the supervisor.
