The Advanced Practice Provider will perform comprehensive inpatient evaluations and manage acute and chronic heart failure patients, including those requiring advanced therapies like LVAD and ECMO. They will collaborate with a multidisciplinary team to develop treatment plans, monitor hemodynamic data, and provide patient education to support transitions of care.
Candidates must hold a Master’s or Doctoral degree as a Nurse Practitioner or a Master’s degree as a Physician Assistant, along with current NYS licensure and national board certification. Preferred qualifications include at least two years of experience in cardiology, heart failure, or ICU settings, with specific expertise in advanced cardiac therapies.
