Hospice RN Case Manager

Optimal Care seeks a Hospice RN Case Manager in Monroe, Michigan. This full-time role offers a salary range of $80,000 to $85,000 annually. You'll provide comprehensive nursing care and case management to terminally ill patients in home or facility settings. The schedule is Monday through Friday 8am-5pm with required rotating weekends.
