Key Responsibilities

• will do Administers the care coordination plan to assess patient needs and ensure seamless transitions between different care settings.

• conduct an evaluation of member's needs and benefits.

• conduct video conferences, share files, and collaborate on patient care plans.

Requirements

• 5 years of direct clinical practice experience post masters degree, e. g., hospital setting or alternative care setting such as ambulatory care or outpatient clinic/facility

• Case management and discharge planning experience preferred

• Managed care/utilization review experience preferred

• Crisis intervention skills preferred
