The Nurse Practitioner will provide medical care and manage terminal illnesses for patients in compliance with the Nurse Practice Act. They will coordinate with the interdisciplinary team to develop individualized care plans and conduct face-to-face hospice eligibility assessments.
Candidates must hold a master's degree and have at least one year of experience as a Nurse Practitioner, along with one year of hospice or palliative care experience. A current state nursing license and accreditation from an approved certifying body are mandatory.
