Provide one-on-one physical therapy to older adults in their homes or senior living communities using the Geriatric House Calls model. Develop individualized plans of care, document effectively in the EMR, and collaborate with an interdisciplinary team to promote patient mobility and independence.
Must hold a valid Physical Therapy license or be eligible to obtain one in the state of practice. Requires a degree from an accredited Physical Therapy program, basic computer proficiency, and current CPR certification.
