About the Role

Join PrimaryOne Health as a Behavioral Health Outreach Specialist in United States. Job DetailsJob Location: Columbus, OH 43205Do you want to make a difference every day? Are you passionate about providing excellent customer service and patient care?

Key Responsibilities

Provides highest level of patient care as a bridge between patients with Behavioral Health needs and their Behavioral Health and medical provider.

Coordinates with primary care providers and behavioral health clinicians to facilitate planning for patient admission to appropriate level of care.

Tracks caseload of patients receiving MAT services both at PrimaryOne Health centers and at collaborating agencies.

Attends community meetings as assigned by Director of Behavioral Health and Social Work.
