Key Responsibilities

Deliver individualized physical therapy treatment plans to patients in their homes under the direction of a Physical Therapist. Collaborate with an interdisciplinary team to ensure coordinated care, accurate documentation, and optimal patient outcomes.

Requirements

Must be a graduate of an accredited PTA program with a valid Illinois license and BLS certification. Requires a minimum of 2 years of clinical experience and a valid driver's license for travel.
