Key Responsibilities

• Design and develop pharmaceutical programs aligned with population health goals and healthcare policies.

• Collaborate with healthcare providers, including physicians, nurses, and other specialists, to optimize medication therapy plans for specific patient populations.

• Analyze population health data to identify medication

• related trends, gaps in care, and opportunities for intervention.

Requirements

• Provide leadership to the pharmacy team, offering guidance, training, and support to enhance their understanding of population health principles and practices.

• Establish and monitor key performance indicators (KPIs) to evaluate the effectiveness of pharmaceutical programs and interventions. MINIMUM QUALIFICATIONS AND REQUIREMENTS:

• Doctor of Pharmacy (Pharm. D.) degree from an accredited institution.

• Current pharmacist licensure in applicable state or region.
