Key Responsibilities

Design and develop pharmaceutical programs aligned with population health goals and healthcare policies.

Collaborate with healthcare providers, including physicians, nurses, and other specialists, to optimize medication therapy plans for specific patient populations.

Analyze population health data to identify medication

related trends, gaps in care, and opportunities for intervention.

Requirements

Provide leadership to the pharmacy team, offering guidance, training, and support to enhance their understanding of population health principles and practices.

Establish and monitor key performance indicators (KPIs) to evaluate the effectiveness of pharmaceutical programs and interventions. MINIMUM QUALIFICATIONS AND REQUIREMENTS:

Doctor of Pharmacy (Pharm. D.) degree from an accredited institution.

Current pharmacist licensure in applicable state or region.

Work Schedule
