Key Responsibilities

The Behavioral Health Consultant identifies and addresses psychosocial factors impacting physical health through brief interventions and patient-centered treatment plans. They also connect patients experiencing homelessness to essential community services and collaborate within a multidisciplinary primary care team.

Requirements

Candidates must hold a Master’s degree in Counseling or Social Work, or a Doctorate in Psychology, along with a valid Arizona clinical license (LPC, LCSW, or Psychologist). A minimum of two years of healthcare experience and an Arizona Department of Safety Clearance Card are required.
