PCHC’s Care Management team is seeking a Social Worker/Care Manager (MHRT/C or eligible) to join our mission-driven, interdisciplinary care teams.

Requirements

• MHRT-C required (or eligibility for MHRT-C) for Adult Care Managers.

• Minimum of three years direct experience in social work in a health care setting required.

• At least one year of case management experience required.

• Having a valid driver’s license with an acceptable driving record and/or an appropriate length of driving experience for insurance purposes is required.

Benefits & Compensation

• Salary range: $49,000 to $70,000 annually.

• PSLF eligible (Public Service Loan Forgiveness).

Work Schedule

• Full-Time position.

• Flexible scheduling available.
