Join Easter Seals Blake Foundation as a Behavioral Health Clinician – Case Management in United States. Description Easterseals Blake Foundation started as the Cerebral Palsy Foundation of Southern Arizona. Since then, we have expanded our services to better serve our community members.

Key Responsibilities

and Responsibilities: Assesses the member’s behavioral health needs and eligibility for services.

Assists staff in developing and monitoring member service plans.

Provides comprehensive intakes, assessments, annual assessments, and offers therapeutic and support services.

Screens, evaluates, and schedules referred members who may need behavioral health services.

Requirements
