Join Carda Health as a Cardiologist – Telehealth Cardiac Rehab in United States. About Carda Carda Health is building the world’s first patient-centered virtual heart and lung platform, starting with cardiac and pulmonary rehab. With only 10% of eligible patients accessing rehab, we’re revolutionizing care through accessible, effective, and engaging home-based programs.
Key Responsibilities
- will be charged first and foremost with overseeing initial evaluations (asynchronous or synchronous) with our patients as well as providing additional oversight (monthly treatment plans, referral for RPM as needed).
- will Join our medical group as an independent contractor Provide high-quality clinical care utilizing best practices and policies set forth by Carda’s clinical leadership team Perform asynchronous or synchronous visits with patients on a schedule that suits your schedule Perform health histories, evaluate patient records, diagnose, and refer to our programs when appropriate Provide lifestyle recommendations to patients related to specific goals Input notes and all other relevant information into our EMR Communicate and partner with other members of the patients’ care team to properly coordinate care and oversee patient care plans on an ongoing basis Join in occasional meetings and participate in a QA program What we look for Board certified in Family Medicine, Internal Medicine, Cardiology, Pulmonology Ideally licensed in five or more of the states.
