Key Responsibilities

  • Evaluate patients’ physical conditions, limitations, and rehabilitation needs
  • Develop and implement individualized treatment plans
  • Provide therapy in accordance with the physician’s Plan of Care
  • Supervise and coordinate care with PTAs and HHAs

Requirements

  • Licensed Physical Therapist (PT)
  • Minimum 1 year of PT experience preferred
  • OASIS and WellSky (Kinser) experience preferred (training provided)
  • Current CPR certification
