About the Role

Provide virtual behavioral health and substance use care management for adults. Deliver patient-centered care by phone or video to improve health outcomes.

Key Responsibilities

• Coordinate care plans with members and care teams.
• Assist members navigating complex medical and behavioral health conditions.
• Partner with local providers for coordinated care.
• Provide compassionate follow-up and case management.
• Coordinate clinical and social resources.

Requirements

• Bachelor of Science in Nursing (BSN).
• Current CCM certification.
• 5+ years nursing experience.
• 2+ years in care management and behavioral health.
• Active Compact RN license in a compact state.
• Willingness to maintain multi-state licensure.
• Work hours 9am-6pm PST.
• Spanish speaking desirable.
