About the Role
Provide virtual behavioral health and substance use care management for adults. Deliver patient-centered care by phone or video to improve health outcomes.
Key Responsibilities
- Coordinate care plans with members and care teams.
- Assist members navigating complex medical and behavioral health conditions.
- Partner with local providers for coordinated care.
- Provide compassionate follow-up and case management.
- Coordinate clinical and social resources.
Requirements
- Bachelor of Science in Nursing (BSN).
- Current CCM certification.
- 5+ years nursing experience.
- 2+ years in care management and behavioral health.
- Active Compact RN license in a compact state.
- Willingness to maintain multi-state licensure.
- Work hours 9am-6pm PST.
- Spanish speaking desirable.
