Provide virtual behavioral health and substance use care management for adults. Deliver patient-centered care by phone or video to improve health outcomes.

Key Responsibilities

Coordinate care plans with members and care teams.
Assist members navigating complex medical and behavioral health conditions.
Partner with local providers for coordinated care.

Requirements

Bachelor of Science in Nursing (BSN).
Current CCM certification.
5+ years nursing experience.
2+ years in care management and behavioral health.
Active Compact RN license in a compact state.
Willingness to maintain multi-state licensure.
Work hours 9am-6pm PST.
Spanish speaking desirable.
