The Social Worker will develop, provide, and evaluate individualized social work assessments and counseling services for hospice participants and their families. They will work collaboratively with the Interdisciplinary Team to support patients through life closure and the grieving process.
Candidates must hold a graduate degree in social work or a related field and possess current licensure in the state of service. A minimum of two years of experience in hospice, oncology, geriatrics, or community health is required.
