Key Responsibilities

The Physical Therapist provides direct patient care, evaluates functional needs, and designs personalized treatment plans in a home health setting. They also document patient progress and provide leadership to support staff to ensure high-quality care outcomes.

Requirements

Candidates must be graduates of an APTA-approved physical therapy program and hold a current, unrestricted PT license. A minimum of one year of experience in a healthcare facility and the ability to travel within the coverage area are required.
