Case Management Social Worker - Epilepsy (Neuro Expansion)

Key Responsibilities

The social worker will assess and plan interventions to help patients and families navigate complex medical and psychosocial needs in both inpatient and outpatient settings. They will also collaborate with multidisciplinary teams to provide counseling, crisis intervention, and care coordination while maintaining community resource networks.

Requirements

Candidates must hold a Master’s Degree in Social Work from a CSWE accredited program and possess current state licensure as a Licensed Social Worker (LSW). A minimum of two years of experience in a hospital or healthcare setting is required, with psychiatric social work experience strongly preferred.
