About the Role

Join RiseWell Community Services as a Registered Nurse - E Full Time in United States. RiseWell Community Services is looking for a full-time Registered Nurses to join our Mobile Residential Support Team. Home office is in West Babylon, NY.

Key Responsibilities

will also work to reduce health gaps and disparities caused by environmental, physical, and social determinants of health by developing health care plans, providing nursing services, and referring patients with social and emotional needs to other community resources.

provide interventions that are supportive to or restorative of life and well-being.

manage health conditions and prevent escalation and unnecessary hospital admissions.

Requirements
